Our software report today says number of patients been seen pre COVID was 3200 patients per year and this number of patients we full filled our UDA target 15,000 without COVID restrictions. Now asking to see 40% of contract valve which will be more patient than this and produce 25% UDA. |

ACORNS- These will be contractual obligations (so failure would give rise to the usual rights/remedies for breach) but there will be no additional payment attached to them (although failure to comply with this ACORN requirement may also adversely impact on the level of payment available for the new metric relating to existing patients). – Does this mean below 100% we will be penalised given that Wales average is circa 90%
We have invested in a mentor scheme to aide recruitment in Wales, can/will any allowance be made for these dentists who will not yet be able to work at the same pace as an experienced dentist?
What if we see more “new patients” can this be deducted from “existing patient” target?
Can we choose a mix of reform and UDA, for example if we have a £340k contract can we elect to keep £170k on UDA and the other £170k reform or does it have to be one or the other?
Q21 from FAQ. Where metrics are not met will this be clawed back, is that ¼ or annually, if ¼ is there the opportunity to “catch up” later in the year which would see payments reimbursed/reinstated? Will contract payments be made at 1/12th from April?
Q17 from FAQ. What about the philosophy of care pathways?
Work is ongoing with the pathways that are designed to be supportive for clinicians and facilitate good common sense dentistry – when can we expect these pathways? Before 1st April?

If an associate has an allocation of 4000 UDA ( example ), based on the new reform contract they see their share of new patients ( 153 pt ) and do their 25% of the UDA’s ( that’s 1000 UDA ), how will practice owners get them to work at the same enthusiasm and speed to do more NHS dentistry !?
What would stop them saying I am not doing NHS anymore, I have done my target !
What is the incentive to do more work ?

Rupert | Nute | Charles Street Dental Surgery | Regarding number of new patients to be seen: Based on the example given in the DCDO letter of 260 new patients/year for a contract value of £170k, if the practice had a number of historic patients higher than the expected amount of 1280, will this correspond to a reduction in the number of new patients expected to be seen?
e.g. if a practice with a contract value as above had 1540 historic patients, would they still need to see such a large number of new patients?
Also, it says "It is not expected that practices will see all historic patients in any 12 month period", yet we are expected to do an ACORN once a year, so it would appear this is not the case.
Thirdly, the criteria for "new patients" has changed significantly - it used to be someone not seen within 2 years. |

Will our overall contract value return to its original 100% value, and all metrics/performance indicators work toward practices achieving that contract value or will practices' maximum contract value continue operating at 90%?
Q2. Re 'supply of mandatory GDS to (and completion of ACORNS for) existing patients (40% contract value).As 'existing patients' are any patients with submitted FP17W in previous 4 financial years, what % of this group would we be expected to submit ACORNS for in 1 financial year? My concern is that some practices e.g. city centre practices serve a much more mobile population (students, workers etc)? will this KPI disadvantage those practices? |

My figures show that in 2019-2020 approximately 88,000 to 176,000 surgery hours were spent completing ACORN paperwork across Wales. That meant 350,000 to 700,000 patients were denied appointments as Welsh dentists were too busy doing ACORN paperwork.
Dental surgery time would be better spent on seeing patients, not filling out XS paperwork.
Why should Mixed practice owners who declined the UDA uplift, be forced to complete ACORNS after April 2022?
They will not be paid for the extra time it takes them.
Dentists struggle with bad backs and RSI (repetitive strain injury)....
having them sit at a computer filling out 70mins of extra NHS paperwork per day is not helpful.
It is demoralising.
Most dentists feel the data is meaningless and of no obvious help to the patient in front of them.
Welsh government have an NHS access issue.
If the ACORN is removed Welsh government will free up space for dentists to see more patients.
Fix the access NHS dental access problem by stopping the ACORN. |

The FAQ section doe not address this fully nor reassure me. My reading is that stable and low need areas will be far more able to meet the patient number based metrics. High need areas with thousands of patients who have not been seen in years, on top of hundreds of new patients will create a huge amount of work.
Yet again high need areas seem to be under supported, going against supposed plans to treat those most in need.

If we see a new patient for an urgent course of treatment but they fail to return for an examination & ACORN, will they be counted? And against which target?
If only new patients count if they have both an ACORN & Examination, does WG realise that practices will no longer offer urgent care appointments as urgent new patient cases will not count as one of the 260 new patients? Have you discussed this with the Health Boards & what was their view?
Will there be the customary 5% tolerance to the UDA only contract as in previous years?
What modelling has WG done to support its calculations?
Will WG exclude patients for whom no previous ACORN &/or Examination has been done in the past 4 years and consider them as New Patients for the purposes of the Reform project?
Taking into account annual leave & study days, what will be the true number of New Patients that a practice will have to see each week? Isn't WG being disingenuous by calling it 5 New patients per week?
Will practices be able to ask HBs to substitute a referred New Patient who, unbeknown to the HB, does not qualify as a New Patient because they had been seen in the previous 4 years?
Recall Intervals: setting the time interval at a minimum of 1 year will be very problematic for practices: It means that patients will be recalled after 1 year to ensure that a gap of 364 days between recalls doesn't occur. Why has WG not taken the pragmatic step of setting the minimum recall interval for 3G patients at 11 months? |

The ones that will not be seen in time will be the historic regular attenders. This seems perverse to reward those that only attend irregularly, it will work in the short term but is unsustainable, where is the incentive for practices to stay within the NHS? |

We would be 6 months behind target before we had even started. This is the end of continuing care for our regular patients, even with their own separate target it will be a check-up every 2-4 years. |

Improving access I agree is an issue. We are concerned that due to our practice being majority private and us agreeing to hold numerous/additional NHS access sessions within the week, this will impede our ability to allocate enough NHS time to achieve the stated 'New patient' target especially considering that we practice in an area with 'high dental care need'.
Achieving a 'balance' between our current patient base/the increased number of emergency appointments post Covid/our NHS access sessions and the access and care of new patients often with high treatment need seems impossible to achieve.
Achieving a 'better access' record is one thing - ensuring that these accessed patients then achieve the best care is a different issue.
We are in two minds which option that would work better for us and our patients. |
